Diver Medical Participant QuestionnaireWe all know that scuba diving requires good physical and mental health. That there are some medical conditions which can be hazardous while diving which may affect you, and in case of a rescue scenario, can put your buddy at risk. Those who have, or are predisposed to any of these conditions, should be evaluated by doctor specialising in Diver Medicine. The UK Diving Medical Committee - UKDMC is a good start point. This Diver Medical Participant Questionnaire helps you to determine whether you should seek out that evaluation. Answering "Yes" to any of the mandated questions identified with a red asterisk (*) will require you upload a copy of a valid and in-date Fitness To Dive medical form. If you have any concerns about your diving fitness that is not represented on this form, consult with your dive doctor before diving. If you are feeling ill, avoid diving. If you think you may have a contagious disease, protect yourself and others by not participating in dive training and / or dive activities until you are healthy again. For your safety, and that of others who may dive with you, please answer all questions honestly. Answering yes to one of more may prompt you to obtain a Fitness To Dive certificate that you can share with the club. |
